Magnus Health

Padua Academy uses the Magnus Health electronic system to collect, track, access, and protect vital health information for all of our students.

In early May, parents/guardians receive welcome emails from Magnus with user names and passwords for each student to log in and complete the health information requirements. All information must be submitted to Magnus via fax, mail, or uploading via a computer by August 1.

Each parent is required to complete or submit the following through Magnus Health. All forms are accessed and printed from Magnus.

  • Vital Health record questionnaire
  • A completed DIAA physical form (required by Padua Academy for incoming students whether a student is an athlete or not. This form must be signed by a health care provider.)
  • Immunization form or proof of up-to-date immunizations as required by the  Catholic Diocese and the State of Delaware (which now includes two varicella vaccines or proof of disease)
  • Over-the-counter medication permission forms for school and field trips

If applicable, the following forms should also be completed:

  • Prescription medication form (for medications taken during school day)
  • Inhaler release form
  • Epinephrine release form
  • Diabetes action plan
  • Seizure action plan
  • Athletic forms
  • Field trip forms
